I. Receiving and filling in the form

3. Collection of application form and labor cost

The operators of the insured units are requested to collect the completed City Social Security (Citizen) Card Application Forms for new participants before the 19th day of the following month of the registration, and at the same time collect the labor cost of the Social Security (Citizen) Card of RMB 20 yuan per person on behalf of the insured.

Four, hand in the form, pay the fee and receive the card

Medical insurance account bank will notify the insured units in the insurance registration after the 19th day of the next month, go to the Guangzhou Municipal Medical Insurance Center for handing in the form, pay the fee and receive the card procedures. V. Issuance of card

The operator of the insured unit shall issue the social security (citizen) card to the insured within one week after receiving the card.
