First, the original insurance agency printed the Maternity Insurance Certificate for eligible employees (hereinafter referred to as the Certificate). Individuals or new employers hold the Certificate of Insurance, apply for insurance and transfer at the designated window of the new insurance agency, and fill out the Application Form for Transfer and Continuation of Maternity Insurance Relationship (hereinafter referred to as the Application Form). Individuals shall provide identification materials such as resident identity cards or social security cards, and agents shall also carry agent identity cards.

Second, after the new insurance agency accepts the application, it will contact the original insurance agency within 10 working days from the date of acceptance, and generate and issue a contact letter for the transfer and continuation of maternity insurance relationship (hereinafter referred to as the contact letter).

3. The original insurance agency shall fill in the Information Form of Maternity Insurance for Insured Persons (hereinafter referred to as the Information Form) within 0/5 working days from the date of receiving the Contact Letter, and send the new insurance agency by registered mail.

Four, in the form of the overall migration of the unit to implement the transfer of maternity insurance for employees, the original insurance agency shall issue a personal "certificate of participation" and "information form", and shall also issue an annual "schedule of maternity insurance payment for the overall migration unit" for the unit, which shall be presented by the migration unit when handling the insurance in the new insured place.

Remarks: If an employee transfers the maternity insurance relationship due to normal job transfer or changes in labor relations, the employer shall go through the relevant formalities at the maternity insurance agency within 30 working days. The payment time for employees to participate in maternity insurance or maternity medical expenses in different co-ordination areas in the province, and the working time for the implementation of maternity welfare system in the original insured place, the co-ordination areas shall be mutually recognized, and the payment time (working time) that meets the requirements can be calculated continuously.
