(2) 4 copies of the unified format of the Social Security Bureau, the certificate of unemployment;

(3) two copies of the unified format of the Social Security Bureau, the unemployment registration form;

(4) one copy of the uniform format of the Social Security Administration, the company to the Social Security Administration routine official documents.
