What are the insurance items of Gusu Social Security Card issued by Jiangsu Bank?
Social insurance card (IC card) is jointly issued by Suzhou Labor and Social Security Bureau and Suzhou Commercial Bank, and issued by Suzhou Social Insurance Fund Management Center. This card is the medical certificate of the insured employee, which is mainly used to record personal account funds and medical expenses. The insured shall hold his social insurance card when handling the following business:
1. Go to a designated medical institution or a designated retail pharmacy for medical treatment and medicine purchase;
2 to the social security agencies for outpatient specific projects or transfer, residence of medical examination and approval procedures;
3. Go to the social security agency to handle the reimbursement procedures for sporadic medical expenses that meet the requirements;
4. When the medical insurance relationship is transferred or terminated for various reasons, the social insurance agency shall go through the formalities of clearing and transferring the Social Insurance Card and personal account;
5. Go through the formalities for changing personal basic information such as name and ID number;
6. Insured employees shall go through retirement procedures;
7 to the social security agencies or designated medical institutions to inquire about my medical insurance payment, personal account, medical expenses, etc.
Second, matters needing attention
1. The social insurance card is only used by the insured employees themselves, and shall not be lent or transferred to others for use, nor shall it be used as another person's social insurance card for medical treatment.
2 "Social Insurance Card" shall be properly kept by the insured employees themselves, and shall not be bent, folded, scratched or contacted with high-temperature and high-magnetic objects.
3. If the employer and employees fail to pay or interrupt the payment of medical insurance premiums on time, the Municipal Social Security Center will freeze the employee social insurance card. During the freezing period, except that the balance of personal accounts in previous years can continue to be used according to regulations, medical insurance benefits will be suspended. The medical expenses incurred during the shutdown period shall be jointly borne by the employer and the employees. Units or employees must pay back the unpaid medical insurance premiums or continue to pay according to regulations after the payment is interrupted, and the social insurance card can be thawed and restored.
4. If the "social insurance card" is lost or damaged and cannot be used, employees should immediately report the loss to the outlets of city commercial banks with valid certificates such as resident ID cards to avoid unnecessary losses.
