Legal analysis: 1. The applicant must provide a 2-inch recent bareheaded color photo with a white background and paste it in the photo frame.

2. Adult application form, with a copy of the front of the ID card on the left and a copy of the back on the right. For the application form for minors, paste a reduced version of the household registration book on the left and a copy of the guardian's ID card on the right.

3. When filling in the application registration form, please don't alter or fold it, keep it clean and tidy, and sign it by yourself or your guardian.

4. The form shall be sealed in the lower right corner, sealed by the unit or village committee, and signed by the reviewer. Business card printing: the applicant or guardian fills in the social security card application registration form and returns it, and the staff scans and uploads the information to print the business card. Card collection: the applicant or guardian brings valid certificates such as ID card or household registration book to the social security card management center to collect the card, check the information on the card surface, and sign for it after confirmation.

Legal basis: Article 7 of People's Republic of China (PRC) Social Insurance Law. The administrative department of social insurance in the State Council is responsible for the national social insurance management, and other relevant departments in the State Council are responsible for the relevant social insurance work within their respective functions and duties. The social insurance administrative department of the local people's government at or above the county level shall be responsible for the social insurance management within its administrative area, and other relevant departments of the local people's government at or above the county level shall be responsible for the relevant social insurance work within their respective functions and duties.
