The process of handling the chronic disease card is as follows.

1. The application time is from March 1 day to March 15 every year. After submitting the application to the medical insurance center, the results will be announced within 15 working days. Applications usually last for one year. This does not mean that you will always enjoy the subsidy after applying, and you have to reapply every year. The application time varies from place to place, so consult the medical insurance center.

2. Application conditions. In China, 25 diseases are classified as chronic diseases, and all of them can be applied. Please refer to the latest list of chronic diseases for specific diseases.

3. To apply for a chronic disease card, you need to prepare a copy of your ID card, social security card, hospitalization medical records, diagnosis certificate, discharge summary and related inspection materials, especially the diagnosis certificate of an authoritative hospital. Otherwise, you can't apply for a chronic disease card. Prepare two recent photos.

4. Fill in the application form. If it is residents' medical insurance, ask the neighborhood Committee. If it is the new rural cooperative medical system, go to the designated medical institution to get the form, fill it out according to the form, and then you can apply.
