How to write the application form for social security payment

The application form for an employee's social security should include such matters as: gender and age; the name of the employer, the employee's job, his or her entry time; the reason for applying for social security registration; and the time of application. China's Social Insurance Law stipulates that employers should apply for social insurance registration for their employees with the social insurance agency within thirty days from the date of employment.
