◆ Application conditions

The business license of the unit is cancelled, revoked or dissolved, bankrupt, revoked, merged, moved to other provinces and cities, etc.

◆ Handling materials

1, original social insurance registration certificate (new enterprises, farmers' cooperatives and individual industrial and commercial households with employees need not be provided after 20 16 10);

2, according to the different situation of the unit, also need to carry the following materials:

(1) Copies of documents for cancellation or revocation of registration license and moving to other provinces and cities;

(two) a copy of the documents approved by the relevant departments for dissolution, cancellation and termination;

(3) The bankrupt unit shall carry a copy of the document officially declared bankrupt by the people's court.

Note: A copy of the materials shall be stamped with the official seal.

◆ Processing flow

1. If it meets the handling requirements, the agency shall take back the original social insurance registration certificate and print the Verification Sheet in duplicate, one for the staff and one for the agency.

2. If the materials are incomplete and can be filled in, the agency shall print the acceptance receipt in duplicate, which shall be signed by the clerk, and each agency shall hold one copy. The handling agency will return all materials.

3. If the handling institution does not meet the handling requirements, it shall print the handling receipt in duplicate, which shall be signed and confirmed by the handling personnel, and each party shall hold one copy. The handling agency will copy all the materials and return them.

If you use the social insurance payment (special) card to pay social insurance premiums, you need to fill out the "Application Form for Transfer and Cancellation of Social Insurance Payment (special) Card" in triplicate (which can be collected free of charge at the agency), and go through the cancellation procedures of the special payment card at the designated outlets of ICBC after being confirmed by the agency seal.

Note: If the social insurance premium is paid by entrusted collection, the authorization termination formalities shall be handled in accordance with the regulations of the bank where the settlement account of the unit bank is opened.
