How to deal with the loss of enterprise social security card?

After the social security card is lost or stolen, the cardholder shall immediately apply for reporting the loss. Loss reporting is divided into oral (temporary) loss reporting and written (formal) loss reporting.

1. Oral (temporary) loss reporting can be handled through the outlets of the Ministry of Human Resources and Social Security, "12333" telephone and online service channels, or through the outlets of cooperative commercial banks, telephone banking and online banking channels.

2. Written (formal) loss reporting shall be handled at the human resources and social security department and the cooperative commercial bank outlets respectively.

Social security card replacement conditions:

1, the cardholder accidentally lost his social security card;

2. The social security card face is not fully stained and cannot be identified;

3. The social security card cannot be read and written normally on the card reading device.

Supplementary information:

1, ID card;

2. Digital photo receipt (not required if photo receipt has been provided);

3, "social security card replacement (loss reporting) application form";

4. Original social security card (conditions 2 and 3 must be met).

Reissue process:

Method 1: The insured shall go through the formalities at the social security institution or social security cooperative bank with the above information.

Method 2: After logging into the enterprise non-CA authentication system, the company applies for a new card number on the "Application for Business Card Form" page of "Social Security Card Business", then enters the "Print Business Card Form" page to print the certificate form, and then submits it to the business card printing window together with the receipt of digital photos and a copy of ID card to reissue the social security card.
