Step 2: Apply for confirmation. Within working days, the insured person shall bring the application materials to Harbin Outpatient Chronic Disease Identification Hospital for declaration and identification according to the procedures specified by the hospital.

Step 3: Query the results. The insured person can go to the reporting hospital to check the appraisal results three working days after the declaration.

Step 4: Enjoy the treatment. The patients who have successfully identified chronic diseases in outpatient department take social security cards to Harbin designated hospitals for basic medical insurance and retail pharmacies for chronic diseases in outpatient department to purchase medicines for treatment. When the expenses are settled, the expenses in line with medical insurance coverage shall be paid by the overall fund according to the prescribed proportion, and the rest shall be borne by the individual.
