How to write the personal social security commitment letter

I, XXX (ID number:), joined Chongqing Co., Ltd. (hereinafter referred to as the company) on, and when I went through the employment formalities, the company clearly told me that I would transfer the social security relationship to the company before the month I joined the company, and the company would pay social insurance for urban workers for me according to policies such as the Labor Contract Law and the Social Insurance Law. However, due to my personal reasons (because my (professional title and name) certificate is attached to the company, my social security is paid by the company now, and the social security relationship has not been transferred to the company, which leads to the company's failure to insure me normally.

For my personal reasons, I voluntarily gave up buying social insurance for urban workers. In order to clarify my responsibilities, I promise:
