1. industrial injury report and medical regulations

(1) If an employee's work-related injury is identified as a work-related injury or suffers from an occupational disease, the employing unit shall apply to the social insurance administrative department for work-related injury identification within the relevant time limit. Within one working day when the insured employee has an industrial accident, fill in the industrial accident declaration form and report it to the social security department for the record; If the industrial accident declaration form cannot be submitted in time, you can report to the social security department of the insured place by telephone within one working day after the industrial accident, and then you must submit the industrial accident declaration form within five working days after the industrial accident.

(2) If an injured worker needs to apply for medical treatment in a medical institution other than the agreed medical institution due to work-related injury, the applicant shall fill out the Application Form for Transfer of Injured Workers in Qingyuan City. If it meets the requirements, it will be issued to the applicant with the consent of the social security agency and official seal.

(3) Where an injured worker applies for medical, physical, psychological and vocational rehabilitation, the applicant shall provide the Application Form for Rehabilitation of Injured Workers signed by the medical (rehabilitation) institution and sealed by the unit. If it meets the requirements, it will be issued to the applicant with the consent of the social security agency and official seal.

(4) If the injured workers need to continue treatment (including recurrence of old injuries) after assessment of disability level, the applicant shall provide the Application Form for Treatment of Recurrence of Old Injuries of Injured Workers, which is signed by the medical institution, sealed by the unit and sealed by the labor ability appraisal institution. (8) Regular tax invoices for transportation, catering and accommodation required for medical treatment outside the overall planning area.
