If the card is defaced, incomplete, unrecognizable, unable to be used on the card reader, or the five basic information of the social security card has changed, you can apply for card replacement.

Treatment conditions

Meet one of the following conditions, you can apply for card replacement:

1. The information on the card surface cannot be recognized because the card is stained or incomplete, so it cannot be used on the card reader.

2. The five basic information on the social security card, namely, name, gender, nationality, date of birth and social security number (except photos) have changed, and the medical insurance information has changed correctly.

Treatment process

1 The insured person fills in the application form at any business window of the social security card service outlets within the scope of this Municipality, and submits the certificate materials and application fees for handling.

2. After the card replacement business is completed, get the Receipt of Beijing Social Security Card Business and the Certificate of Renewal of Social Security Card, and get a new social security card according to the card date on the card certificate.

3. The person who changes the social security card will get a new card with the supporting materials and the voucher for replacing the new social security card.
