1. Provide the completed Application Form for Social Insurance Retirement (Transfer) of Enterprise Employees in Shenzhen or the resignation certificate issued by the original work unit (both the application form and the unit certificate shall be stamped with the official seal of the unit or the special seal for personnel).
2. Show the original personal ID card and submit two copies (if it is a second-generation citizen ID card, you must also copy the back of the ID card).

7. Surrender shall be handled by the insured employee himself and shall not be collected on his behalf.
