1. After receiving the form and filling in the registration form, the agent of the insured unit copies the application form of social security (citizen) card on both sides and sends it to each new insured person, and instructs the new insured person to fill in the application form according to the requirements of the application rules.

2. Before the 3rd day of the next month of insurance registration, the agent of the insured unit will arrange for the new insured to take his ID card (or military officer's card and passport) to the photo studio designated by the social security (citizen) card to take digital photos of the social security (citizen) card, and paste the photo receipt in the corresponding position of the application form. The social security card (citizen card) charges 4 yuan/person for taking photos.

3. To receive the application form and fees, please ask the manager of the insured unit to get the completed social security (citizen) card application form from the new insured person before the next month of enrollment registration 19, and also get the social security (citizen) card fee, 20 yuan/person.

4. Hand in the form, pay the fee and get the card. The bank where the medical insurance account is opened will notify the insured unit to go through the formalities of submitting the form, paying the fee and getting the card in Guangzhou Medical Insurance Center after the second month of enrollment registration 19. 5. The agent of the card-issuing insured unit shall issue the social security (citizen) card to the insured within one week after receiving the card.
