(a) the identity certificate of the motor vehicle driver;

(2) The physical condition certificate issued by the medical institution.

Article 66

Under any of the following circumstances, the driver of a motor vehicle shall, within 30 days, apply to the motor vehicle management office outside the place where the motor vehicle driver's license is issued or issued:
