I actively participated in the prevention and control training of hospital infection, organized department personnel to learn the knowledge of hospital infection, and conducted regular training on gas, hands and things every month. Because of the unreasonable layout of the intervention operating room, it brings difficulties to the management of closing shoes. This year, under the active communication between the director and me, the office area moved out of the operating room, which reduced the phenomenon of people entering and leaving and patients breaking in by mistake. The operating room is in a normally closed state and has achieved ideal management effect. First, the basic situation of the department:
General medical staff ***22 people: 2 doctors, 2 assistants and 3 people without professional titles; Nurse 1 person, nurse 10 person, 4 people without professional title; 2 drivers of "120"; 2 Cleaners: 1 recruiters, 1 temporary workers. This year, I obtained medical practitioners 1 person and nurses 1 person; Qualified nurse 1 person. There are 28 beds in the second ward on the second floor, 4 rescue beds and 6 observation beds in the emergency department on the first floor. Now our department has a debridement room, where we can perform debridement, suture, dressing change, suture removal and small resection, which can reduce the pressure on the surgical ward, ensure the basic needs of emergency patients and facilitate the masses to see a doctor.
Second, the main work:
(1) Business work:
1. At present, the doctors on duty in our department are divided into day shift and night shift, and are on duty 24 hours a day. The main shift is mainly responsible for the treatment of hospitalized patients during the day, and the night shift is mainly responsible for the critical patients at night to ensure that doctors can see a doctor at any time. The deputy physician is mainly responsible for the "120" house call, and is also responsible for debridement and suture (in the absence of the deputy physician, the main shift is responsible).
2. Nurses have at least four shifts during the day and two shifts at night to ensure that patients can stay under observation for infusion for a long time, so that patients can be infused in time. Nurses require light makeup, neat clothes, "seven voices and six hearts" service, and nurses are on duty 24 hours a day, putting an end to the phenomenon of seeing a doctor at night and calling doctors and nurses everywhere.
3. When the emergency building is relocated, there are fixed doctors, nurses and drivers who are responsible for home visits every day, and two ambulances are on standby at the hospital 24 hours a day to ensure that the ambulance phone rings three times and someone answers the phone at any time. Put an end to the phenomenon that patients can't find a driver or wait too long for help in the past. Since May this year, the management of ambulance vehicles has been strengthened. Now it is stipulated that ambulances should be parked in hospitals, and drivers should check the condition of vehicles every day, clean the sanitation inside and outside the vehicles, so as to keep ambulances in a good standby state. Re-establishing the new image of "120" first aid has changed people's views on "120" and the social benefits have been continuously improved.
Based on the above factors, with the joint efforts of all medical staff, the emergency department has been recognized by the society, and the number of patients is increasing day by day, making our department treat 856 inpatients and 806 discharged patients throughout the year; Emergency 1264 person-times, gastric lavage in 20 cases, rescue of critically ill patients in 5 cases, and "120" treatment in 242 cases. The economic and social benefits of our department have increased steadily.
(2) Formulate various systems and post them on the wall, as follows:
1, first consultation responsibility system, responsibilities of doctors at all levels, infectious disease reporting system, medical safety system, emergency room system, gastric lavage room system, debridement room system, etc.
2. Improve the rescue charts of several critical diseases, such as upper gastrointestinal bleeding, left heart failure, shock, arrhythmia, organophosphorus poisoning, etc.
3. Formulate the green channel system and rescue flow chart for hospital rescue of critically ill patients;

(3) Pay attention to ideological and political education and create a good medical safety atmosphere;

Therefore, the department regularly organizes business study, strengthens the training of various first-aid techniques, and forms a good learning atmosphere and mutual communication in the department.

In recent years, new infectious diseases have occurred from time to time, and old infectious diseases have resurfaced. Strengthening the prevention and treatment of infectious diseases is something that every medical worker should attach importance to, and the emergency department bears the brunt. Therefore, only by strengthening the training of infectious disease prevention knowledge and seriously treating every patient, especially those with fever, can we truly find, report, isolate and treat in time and make detailed records. Therefore, the department has repeatedly studied the prevention and treatment of infectious diseases. In addition, according to the unified arrangements and requirements of the health administrative department at a higher level and the county party committee and government, all kinds of adverse vaccine reactions should be registered in detail and dealt with.
(six) improve service attitude, improve service quality and enhance social benefits:
The medical staff in our department establish the idea of "patient-centered", take the satisfaction of patients as the criterion of medical service, and do a good job of "service". Always take patients' wishes as the first signal, patients' satisfaction as the first pursuit, patients' interests as the first goal, always think about patients' thoughts and urgent needs, constantly strengthen service awareness, fully respect patients' requirements for service convenience and humanized communication, carry out "family-friendly" and "humanized" services, and achieve "six hearts and seven voices" smiling service. Before infusion, nurses should give health education to patients and their families, and introduce the hospital environment (such as toilets, safe passages, water dispensers, etc.). If anything happens during the infusion, please ring the bell. After the improvement of environment and the change of service concept, patients are satisfied with the service of emergency department. Now there are more and more patients in our department who come to see a doctor every day, which shows that our service has been recognized by the people of the county and the social benefits of the emergency department have been continuously improved.
Three. Existing deficiencies and rectification measures:
1, the environment of the ward, emergency room and observation room is not clean and tidy, especially after the patient leaves after infusion and needle removal, the bedding and ground garbage are not cleaned in time, resulting in messy rooms. Supervise the responsible nurses and cleaners to patrol the ward at least 3 ~ 4 times a day to ensure that the ward is always in a clean and tidy state;
2. Sometimes doctors' prescriptions are not standardized and the logs are omitted;
3. The intensive care unit is not fully utilized;
4. The nurses in emergency room are too young and lack of first aid experience, so it is difficult to complete the first aid task. The quality of individual nurses is not high, and the concept of sterility is not strong.
5, due to the shortage of nursing staff, basic nursing is not in place, such as first-class nursing patients can not get the service of first-class nurses, second only to daily work.
6. Ward management is not satisfactory.
Over the past year, due to the dean's attention, support and help, nursing work has made some achievements in nursing quality and professional ethics construction, but there are also some shortcomings that need to be further improved.
