What documents do institutions need to provide for physical examination?

(1) On-the-job personnel must provide the proof that their unit agrees to enter the physical examination when entering the physical examination (if there is a competent department, it must be approved by the competent department).

(2) Resignation, the agent (who has gone through the employment formalities) shall provide the certificate issued by the Human Resources and Social Security Bureau of the city or county (district) to which the personnel relations escrow department belongs, agreeing to enter the examination.

(3) When a person who has not been a personnel agent in a private enterprise enters the medical examination, he/she shall provide the certificate of consent to enter the medical examination issued by the unit and a copy of the business license of the enterprise as a legal person.

(4) On-the-job primary and secondary school teachers are allowed to enter the exam, but the unit consent certificate approved by the local county and city education administrative department must be provided during the physical examination.
