In order to register for the 20 14 college entrance examination in Shaanxi Province, relevant matters are hereby notified as follows:

I. Registration conditions

(1) Applicants who sign up for the college entrance examination in Shaanxi shall meet the following basic conditions:

1. Abide by the Constitution and laws of People's Republic of China (PRC);

2. Physical conditions meet the enrollment requirements of ordinary colleges and universities;

3. As of August 3 1 of the year of the college entrance examination, he has lived in Shaanxi for 3 years;

4. As of the year of the college entrance examination, I have been studying in Shaanxi for three consecutive years, that is, from the first school year, I have been studying in the senior secondary education school in our province for three consecutive years, and I have completed my studies according to the regulations and obtained the graduation certificate issued by the senior secondary education school in Shaanxi Province. Ordinary high school graduates should also have provincial high school academic level test scores.

(two) one of the following circumstances, shall not be registered in our province:

1. College students with higher education;

2. Students who have not graduated from senior secondary education schools;

3. Fresh graduates of senior secondary education who registered by deception and illegally participated in the college entrance examination (including the national unified examination, the provincial unified examination and the separately organized college entrance examination, hereinafter referred to as the college entrance examination) in the non-graduation year;

4. Candidates who are suspended from the college entrance examination due to violation of the national education examination regulations;

5. Being taken compulsory measures by the relevant departments or serving a sentence for violating the criminal law.

(3) If a graduate from a senior secondary school with the same academic qualifications signs up for the college entrance examination, he and his father or mother must have a permanent residence in Shaanxi, and the first household registration is in Shaanxi. Before registration, I will write a written explanation of the same academic level, and the education administrative department of the county (district) where the household registration is located will review and issue an opinion. After the public security department issues the household registration audit opinion, the office of the county (district) Admissions Committee where the household registration is located accepts its application for registration.

(4) If the father or mother who goes out to work is studying in other provinces and does not meet the requirements for the college entrance examination in this province, and I and my father or mother have registered for the first time and have a permanent residence in Shaanxi, candidates can sign up for the college entrance examination in the county (district) where the household registration is located after the provincial (district, city) admissions examination institution has issued a certificate that they cannot take the local college entrance examination.

(5) I will normally move my permanent residence into our province with my father or mother because of military placement or job transfer (referring to the transfer handled by state organs, institutions and personnel management departments of state-owned enterprises), and transfer my senior secondary education status to our province, regardless of the age limit of household registration and school status.

(6) Candidates who have been approved to drop out of school in the senior secondary education stage for some reason, but still retain their school status during the suspension period, and have obtained full school status in the senior secondary education stage in our province after resumption of school, can apply for the college entrance examination.

(7) To apply for registration, foreigners living in China shall abide by the Constitution of People's Republic of China (PRC), have the same educational level as those who graduated from senior secondary education schools, meet the enrollment requirements of ordinary colleges and universities, and must hold the Permanent Residence Permit for China People and Foreigners or the Residence Permit for Foreigners issued by Shaanxi Provincial Public Security Department.

Second, the registration time and method

(1) Registration time: 20 13 12 1-7.

(2) All those who plan to take the 20 14 general college entrance examination (including all kinds of test-free candidates, candidates who plan to take various special or separate entrance examinations, etc.). ) must be registered in the county (district) where the household registration is located. Graduates from schools in counties (districts) where they are not registered will register in counties (districts) where they are registered, and the schools and the education administrative departments of counties (districts) where they are registered will register at the designated registration points in counties (districts) where they are registered, 20 14 in Shaanxi Province (see annex 1).

(3) Candidates in juvenile classes shall register with the county (district) where their household registration is located with the Notice of Applying for Schools. Candidates studying in Tibet class in Chinese mainland, our province register at the location of the school.

(4) The subjects applied for are divided into five categories: literature and history, art (literature), science and engineering, art (science) and sports. At the time of registration, candidates can only choose one of them and cannot change it after registration. The national unified examination subjects of literature, history and art (literature) are Chinese, mathematics (literature), liberal arts integration and foreign languages; The national unified examination subjects of science and engineering, physical education and art (science) are Chinese, mathematics (science), science synthesis and foreign languages. Art and sports candidates can only fill in the early admission batch of art and sports volunteers when filling in their volunteers, and may not fill in other subjects; The remaining batches of art (literature) candidates can also volunteer in history, and art (science) and sports candidates can also volunteer in science and engineering. Candidates of literature, history, science and engineering are not allowed to fill in other subjects.

(5) According to the Notice on Applying for the Second Generation Resident Identity Card for the Entrance Examination of Colleges and Universities (Shaanxi Education Examination [2011] No.3), "The second generation resident identity card must be issued for the college entrance examination, otherwise the registration formalities will not be handled. If the normal registration is affected by the failure to issue the second-generation resident ID card, the responsibility shall be borne by the candidates themselves. " Candidates must go to the designated place of the county (district) Admissions Office to collect their second-generation ID card information and get the registration number and password before registration. When registering, log in to Shaanxi Admissions Examination Information Network, check my name, gender, ID number and other information, carefully read the Instructions for Candidates, Measures for Handling Violations in National Education Examination and 20 14 College Entrance Examination Candidates' Integrity Commitment Letter, and enter the registration information after agreeing to perform integrity.

(six) all kinds of special candidates who enjoy the admission care policy shall submit the original relevant documents, fill in the "Certificate Form for Special Candidates" and be audited by the competent department. Candidates who only belong to ethnic minorities and no longer have other care qualifications do not need to fill in this form, and their ethnic minority qualifications are examined and confirmed by the county (district) recruitment office after collecting the candidate's ID card information.

Cities and counties (districts) should strictly examine the qualifications of special candidates and publicize them locally, accept social supervision, and investigate and deal with problems in a timely manner. Except for minority candidates, municipal admissions offices classify special candidates and submit them to provincial admissions offices before the end of May 2065438+2004. The Provincial Admissions Office examines the qualifications of special candidates and publicizes them online in Shaanxi.

(seven) candidates who have been admitted to relevant colleges and universities before the national unified examination for enrollment in ordinary colleges and universities may not take the unified examination again. Before the unified examination, the provincial admissions office will sort out and distribute the list of candidates who have been admitted by the city and county (district) admissions office, and the county (district) admissions office will no longer issue admission tickets for candidates who have been admitted.

(8) In the last year, if a student has been admitted to an ordinary university, but has not been admitted to an admitted institution and has not voluntarily given up studying, he should issue written materials to the county (district) Admissions Office to ask him to drop out, and the county (district) Admissions Office is responsible for reviewing whether the candidate belongs to the situation of "voluntarily giving up studying in an admitted institution for various reasons". Candidates who can't provide written proof materials or are identified as "voluntarily giving up studying in an admission institution for various reasons" will be listed as the lowest in the same score when the results of the college entrance examination are announced on 20 14, and the fact that they voluntarily gave up studying in an admission institution in the previous year will be recorded in the electronic file, which can be used as the reason for the admission institution to quit the file.

(nine) according to the standards approved by the Provincial Price Bureau and the Provincial Department of Finance, candidates must pay the registration examination fee of 95 yuan. The registration fee is paid through online banking, and the registration follow-up procedures such as confirmation can only be handled after the payment is successful. Third, do a good job in registration to ensure the accuracy of candidates' information.

In order to ensure the accuracy of candidates' registration information, cities and counties (districts) should do a good job in all stages of registration in a down-to-earth and meticulous manner.

(1) Candidates are required to complete the online registration procedure from 201312/kloc-0 to 7. All counties (districts) should review the qualifications of candidates in advance, and those who do not meet the registration conditions may not register. Candidates who hold resident identity cards issued by non-county (district) public security organs and non-county (district) student status should be carefully examined. Only the county (district)' s household registration and student status are true and meet the requirements can they register. The county (district) will register these candidates and report them to the Municipal Admissions Office for the record. All registration points should do a good job in registration and publicity. Before registration, it is necessary to debug the information collection equipment of the second-generation resident ID card, and do a good job in collecting, uploading, printing and distributing the registration number and password of candidates. Those who intend to enroll in colleges and universities must report on time, and those who fail to report after the time limit will bear the consequences and may not make up the report.

(2) Before 2013, 12 and 15, confirm the registration information, and conduct ideological and political moral assessment on candidates. After the registration, the county (district) Admissions Office should print out a checklist of candidates' registration information, which will be signed by the candidates themselves for confirmation. The county (district) admissions office should promptly correct the information errors that can be changed during the confirmation of candidates.

Candidates who have applied for the second-generation ID card earlier and whose appearance is quite different from that of the ID card at the time of registration may apply to the county (district) recruitment office for on-site digital photos. At the time of application, the county (district) household registration management department and middle school (non-school students are issued by their work units or township and street offices) must be issued to confirm the identity certificate and the original ID card. After the county (district) admissions office has passed the examination, it will go to the registration site to receive the digital photos of the candidates. The photo quality standard shall be implemented in accordance with the Notice on Implementing Digital Photography for College Entrance Examination (Shaanxi [2006] No.34). Counties (districts) in the process of accepting the application for collecting digital photos on site, we should strictly examine and verify, to prevent "college entrance examination immigrants" and * people from taking the opportunity to muddle through.

The examination of ideological and political morality is based on the actual performance of candidates themselves. The school or unit where the candidate is located is responsible for making a comprehensive appraisal of his political attitude, ideology and morality, and the content should be complete and accurate. The examination of students' ideological and political morality is entered into the online registration system by a special person and audited by the school; The appraisal of non-school students shall be filled in by their work units (filled in by township and sub-district offices if there is no work unit), and the Examination Form of Ideological and Political Morality of Non-school Students in Shaanxi College Entrance Examination 20 14 (see Annex 2) shall be stamped with the official seal; The school is responsible for the identification of registered candidates in the county (district) where the household registration is located. The county (district) Admissions Office shall designate a special person to enter the ideological and political moral assessment content of the candidates who are not in school and the county (district) where the household registration is located into the online registration system. The original evaluation form shall be kept for future reference.

Candidates who have been subjected to criminal punishment, public security administration punishment or disciplinary action shall provide detailed materials such as the facts of the mistakes made, handling opinions, and their own understanding of the mistakes and the realistic performance of correcting them, and shall be responsible for their authenticity.

In any of the following circumstances, the ideological and political moral assessment is unqualified:

1. Oppose the basic principles set by the Constitution or join a cult organization;

2. Violating the Law on Public Security Administration Punishment and being punished by public security administration, if the circumstances are serious and the nature is bad.

(3) Before February 20, 2003, 2065438+2003, determine the test center and examination room, and arrange the candidates' admission ticket number. The admission ticket number shall be randomly arranged by the Municipal Admissions Office through the computer. The admission ticket number of candidates in the same county (district) and the same subject shall not be left blank, and the admission ticket number of students in the same class shall not be continuous. Those who provide convenience for cheating in the exam by arranging the admission ticket number should be seriously investigated for responsibility.

20 13, 13 Before February 22nd, all counties (districts) should complete the printing and distribution of "Textual Research on Examination Report of Professional Courses" for art and sports candidates.

There are 30 candidates in each examination room, and the mantissa examination room with less than 30 candidates in each subject is set up separately, and mixed examination rooms are not allowed. Candidates for Tibetan classes and juvenile classes are arranged in the mantissa examination room. Minority language candidates should be arranged in the mantissa examination room of the same test center.

(5) Have a physical examination for candidates before April 14, 20 10, and the organization of physical examination will be notified separately. The physical examination form and bar code are uniformly printed by Ban Zhao, and Ban Zhao will collect them from Ban Zhao before 20 14.

(VI) Municipalities, counties (districts) shall notify the secondary schools under their jurisdiction to provide students' school registration certificates, ideological and moral evaluation forms and school registration files for the candidates who register in the counties (districts) where the household registration in Shaanxi Province is located in 20 14.

Fourth, strict qualification examination to prevent "college entrance examination immigrants"

All localities should strictly control the qualification examination of candidates, strictly investigate the illegal registration by using fake household registration and fake student status, and improve various effective qualification examination methods such as double identification of household registration, electronic registration of high school student status, verification of household registration book and ID card, and publicity of registration qualification in accordance with the spirit of the Notice of the Provincial Department of Education and the Public Security Department on Doing a Good Job in the Qualification Examination of College Entrance Examination Candidates (No.22 [2005] of Shaanxi Jiaosheng). Education administrative departments at all levels and middle schools should strengthen the management of student status, and it is strictly forbidden to forge student status files. In accordance with the "Regulations on the Management of Middle School Students' Status" formulated by the administrative department of education, we should strictly examine the ways and registration procedures of Shaanxi middle school candidates' status, and resolutely put an end to the illegal application of candidates from other provinces who have come to Shaanxi to attend high school for less than three years. It is necessary to include the academic level examination papers of ordinary high school graduates in the scope of registration qualification examination. Before the registration begins, please ask the local public security department to strengthen the examination of candidates' household registration, focusing on the examination of candidates holding resident ID cards issued by foreign public security organs, and deal with problems in time when found.

All localities should follow the principle of "who is in charge and who is responsible", clarify the responsibility for the qualification examination of the college entrance examination, and implement it to everyone. Give full play to the role of administrative supervision, mass supervision and public opinion supervision, strengthen supervision and inspection of key links, key posts and key time periods, and intensify investigation and punishment of acts of favoritism and malpractice. Staff who organize or participate in the "college entrance examination migration" activities and provide convenience for forging middle school students' school records should be severely punished in accordance with party discipline and government discipline, and those who violate the law should be transferred to judicial organs for legal treatment. Violation cases involving "college entrance examination immigrants" should not only be investigated for the responsibilities of the parties, but also the leaders in charge and the relevant responsible persons.
