Application form,

No criminal notarization (for you and your spouse, for your spouse, there is no criminal record in every country where you lived in the previous 7 years, except New Zealand)

Property security (your spouse's, contract, tax bill)

New Zealand identification of spouse (i.e. PR sticker on passport)

Proof of your relationship with your spouse (marriage certificate and the like, at least 12 months of spouse relationship)

Birth notarization (yours)

Physical examination certificate (including X-ray chest X-ray)

Passport.

Your passport (it should be a public certificate, this is not very clear).

Two passport photos.

The application may take 6-9 months, depending on whether you need to fill in the information.
