Valid passport: If your passport is due within six months of your expected arrival date in the United States If the visa issuance page has expired or is damaged, or there is no more blank visa issuance page in the passport, please apply for a new passport before coming for the interview.

One photo: A 5cm x 5cm square color frontal photo with a white background taken within 6 months. Please attach the photo to the English DS-156 application form with glue instead of staples. See photo requirements for details.

Applicants must present two copies of the original application fee receipt when submitting the visa application. Only applicants eligible for A-1, A-2, G-1, G-2, G-3, G-4, C-3, NATO and U.S. government-sponsored visas are exempt from the application fee.
