Examination application procedure

3. Hold the nurse license form to the unit that issued the registered nurse or professional nurse license. The unit directly sends a copy of the nurse's practice certificate to CGFNS institution;

4. Attach a copy of the high school diploma or high school equivalent certificate;

5. Attach three signed photos and fill in the CGFNS photo appraisal form;
