Submit the applicant's household registration book, resident ID card, birth medical certificate and other proof of parent-child relationship, the original marriage certificate of Hong Kong or Macao parents and a copy, the resident ID card of Hong Kong or Macao parents, a copy of Hong Kong or Macao residents' travel permit to and from the Mainland, and a copy of Hong Kong or Macao permanent resident ID card and foreign passport if the parents are foreign and have no proof of Hong Kong or Macao children. 4. If the applicant is born out of wedlock or the parent-child relationship is in doubt, the parent-child relationship shall be identified in the hospital or institution designated by the exit-entry administration department of the provincial public security organ. 5.
