How do foreigners seek medical treatment in Taiwan Province Province?

Preparations for mainlanders to apply for medical treatment in Taiwan

* Please send medical reports, medical records, abstracts, etc. The feasibility and medical expenses of patients seeking medical treatment in Chinese mainland can be assessed by the center. Only when the center meets the requirements for Taiwanese to come to Chinese mainland for medical treatment can they apply for a medical visa.

Documents required to apply for a medical visa:

1. Please fill in the Application Form for Entry and Exit of Chinese mainland Personnel (attachment 1) in detail and paste it on the front half of the portrait. The portrait was taken recently 1 year, with a length of 4.5 cm and a horizontal length of 3.5 cm. Take off your hat, do not wear colored glasses, and the facial features are clear and uncovered, which is enough to identify people's faces. The length from the top of the head to the chin shall not be less than 3.2 cm and more than 3.6 cm, and the front half of the portrait is thinner on a white background. 2. Copy of Chinese mainland resident identity card, and other copies of fully certified licenses or identity documents.

3. Certification documents such as notarial certificate of kinship between the applicant and his peers.

4. Power of Attorney of Immigration Department (please sign and seal the client)

5. Physical examination certificate of mainlanders (please fill in and sign in detail)

6.SEF power of attorney (Annex II, please sign and seal the client)

7. Letter of guarantee (Annex III, please ask relatives and friends in Taiwan Province Province to be guarantors and fill in the letter of guarantee).
