Documents to be prepared:

5. Qualification documents (copies of the attached documents shall be signed or sealed by the applicant):

Remarks:

1. If the applicant is a student over 18 years old, he/she shall provide a student ID card and proof of attending the school, and if he/she is under 20 years old, he/she shall obtain the consent of his/her immediate family.
