20 19 notice on the work arrangement of teacher qualification certificate identification in Changzhi city, Shanxi province

In order to implement the spirit of Document No.30 of Shanxi Provincial Department of Education [20 19], according to the requirements of People's Republic of China (PRC) Teachers Law, Regulations on Teachers' Qualification and Measures for the Implementation of Teachers' Qualification Regulations, the work arrangements for the qualification of teachers in Changzhi City in 2019 are as follows:

First, the scope of appraisal

(1) Social personnel with Changzhi household registration (including on-the-job and unemployed personnel);

(2) Persons registered in other provinces and cities who hold residence permits in Changzhi City and are within the validity period;

(3) Full-time undergraduate and junior college graduates from full-time colleges and universities in Changzhi, graduate students and full-time fresh graduates from Changzhi Kindergarten Normal School;

(four) hold a residence permit for Hong Kong, Macao and Taiwan residents within five years of validity or a permit for Hong Kong, Macao and Taiwan residents to travel to and from the mainland or a resident of Hong Kong, Macao and Taiwan in mainland travel permit for taiwan residents.

Persons who have not reached the national statutory retirement age and obtained the national primary and secondary school teacher qualification examination certificate may apply for the teacher qualification of kindergartens, primary schools, junior middle schools, senior middle schools and secondary vocational schools.

The following applications for teacher qualifications will not be accepted for the time being: the qualification of internship instructor in secondary vocational schools; The same applicant applies for two or more teacher qualifications in the same year; Being deprived of political rights or intentionally committing a crime and being punished by fixed-term imprisonment or above; If a teacher's qualification is revoked, he shall not re-apply for the teacher's qualification within 5 years from the date of revocation; Cheating in the teacher qualification examination was sentenced to 1-3 years according to the Measures for Handling Violation of National Education Examination.

Second, the appraisal schedule

(1) Online registration: 08: 0019-08: 00 on May 23rd at 08:00 18:00 on May 23rd. Applicants should log on to China Teacher Qualification Network (www.jszg.edu.cn) during the registration period.

(II) On-site confirmation time of teachers' qualifications in high schools and secondary vocational schools: 2019-May 3 1, and May 3 1 is the on-site confirmation time for fresh graduates of Changzhi College.

The on-site confirmation time of junior high school and below teachers' qualifications shall be determined by the county and district education bureaus. Please consult the local education bureau.

(3) The time for obtaining the teacher qualification certificate will be notified separately. Please pay attention to the announcement on the website of Changzhi Education Bureau (www.jyj.changzhi.gov.cn).

Third, the identification conditions

Applicants must hold the certificate of primary and secondary school teachers' qualification examination issued by the examination center of the Ministry of Education within the validity period, and the "teaching subjects" to be applied for should be consistent with the interview subjects.

(A) ideological and moral status

Support the leadership of China * * * production party, love the socialist motherland, adhere to the party's basic line, have good political and moral quality, abide by the Constitution and laws, love education, fulfill the obligations stipulated in the Teachers Law, and abide by teachers' professional ethics.

(2) Educational requirements

1. To apply for kindergarten teacher qualification, one should have graduated from kindergarten normal school or above, or graduated from other colleges or above.

2. To apply for the qualification of a primary school teacher, one should have a secondary normal school diploma or above, or a junior college diploma or above.

3. To apply for junior high school teacher qualification, you should have a bachelor's degree or above in a normal college or other university.

4. To apply for the qualification of a teacher in a senior middle school or a secondary vocational school, one should have a bachelor's degree or above in a higher normal school or other university.

(3) Putonghua requirements

The Putonghua level of applicants for teacher qualification should reach Grade B or above in the Putonghua proficiency test standard issued by the State Language Committee. To apply for the qualification of Chinese teacher, the level of Putonghua should reach Grade 2 or above.

(4) Physical examination requirements

Can meet the needs of education and teaching, and has good physical fitness. The physical examination standards were implemented in accordance with the Notice of the Ministry of Education on Adjusting the Physical Examination Standards for Applicants with Kindergarten Teacher Qualification (J.Z.Zi [2065 438+00] 15) and the Physical Examination Standards and Measures for Applicants with Teacher Qualification in Shanxi Province (Revised) (J.Z.Zi [2005] 19), and the physical examination conclusion was qualified.

Applicants for the qualification of teachers in senior high schools and secondary vocational schools received the physical examination form on the first floor of Physical Examination Department 1 of Changzhi Traditional Chinese Medicine Hospital (200 meters east of the passenger transport center at No.324 Houxi Street, Shifu) from May 7 to 28, and had a physical examination (attached 1 inch photo 1 piece). Junior high school and below teacher qualification physical examination hospitals are designated by county and district education bureaus.

Applicants for teacher qualification shall truthfully fill in the column of "past medical history" in the teacher qualification physical examination form. If abnormal items such as heart, liver, gallbladder, spleen and kidney and suspected respiratory symptoms are found during physical examination, ECG, B-ultrasound, chest X-ray and other related items must be further examined. Applicants can register their personal accounts at any time during the opening period of "China Teacher Qualification Network" (you need to choose the "teacher qualification applicant online newspaper entrance" for registration), and the ID number is personal account. Once registered, it cannot be modified. Please fill it out carefully.

Before the teacher qualification registration begins, the applicant should first improve his personal information and download personal books.

1. Improve personal information

After the applicant logs in with the registered account, click "Personal Information Center" to improve personal identity and other information on this page.

(1) "personally identifiable information". Applicants need to increase their gender and nationality in this column (Hong Kong and Macao applicants can choose specific nationality or others when choosing nationality). The applicant can modify other information except "Certificate Type" and "Certificate Number" on this page.

(2) "Teacher Qualification Examination Information". Candidates who have participated in the national primary and secondary school teacher qualification examination and passed the examination can check their own examination situation in this column.

(3) "Putonghua certificate information". Applicants can add and modify personal Putonghua information in this column.

① Under the category of "Verification Certificate", enter the certificate number and other information, and click the "Verify" button, and the system will verify the Putonghua certificate information in the National Putonghua Proficiency Test Information Management System.

(2) If the information of Putonghua certificate cannot be verified, please check whether the currently verified information is consistent with the "name, ID number and certificate number" in the certificate information.

(4) "Education and student status information". Applicants can add and modify personal education and school information in this column.

① Under the type of "Check Education", enter the number of education certificate and click the "Check" button, and the system will obtain relevant information from the information management system of the National College Student Information Consulting and Employment Guidance Center (Xuexin.com).

② If the academic information cannot be verified, please check whether the information currently verified is consistent with the "name, ID number and certificate number" in the academic certificate information.

③ If the certificate information cannot be verified through the above steps, please select the type of "unable to verify academic qualifications", fill in the relevant information and upload the corresponding electronic certificate (the picture size is less than 200KB and the format is JPG) for background manual verification.

④ For secondary school education such as normal school and kindergarten teacher, please select the type of "education that cannot be verified", fill in the relevant information and upload the corresponding electronic certificate (the picture size is less than 200KB and the format is JPG) for manual verification in the background.

⑤ If the applicant's educational background is Hong Kong, Macao and Taiwan or studying abroad, and it is impossible to verify his educational background, please select the verification type as Hong Kong, Macao and Taiwan or studying abroad, and upload the educational background certificate of Hong Kong, Macao and Taiwan or foreign countries according to step (3). It is suggested that applicants with Hong Kong, Macao and Taiwan education or foreign education should be certified in the "Academic Degree Certification Application System of the Education Service Center of the Ministry of Education"/) in advance.

Step 2 download personal books

Applicants can download personal books from the "Data Download" page on the homepage of China Teacher Qualification Network before the registration is confirmed, and use them when registering.

The downloaded personal handbook is printed in A4 white paper. If you use this book in the application form of teacher qualification certificate, please write it in block letters at the "person" and sign my name. After filling in the signing time at the "year, month and day", take a photo vertically and clearly on the front of the paper and upload it. After signing the personal book, you can see the overall effect when previewing the application form of teacher qualification certificate after successful registration. If the personal book is found in the preview process with incorrect location and unclear signature, it can be uploaded again.

(3) Registration of the applicant

After the online registration begins, the applicant will log in to "China Teacher Qualification Network" with his account within the specified online registration time.

1. Select a certification authority

According to the Teachers Law and the relevant regulations of our province, the qualifications of teachers in kindergartens, primary schools and junior middle schools are determined by the education administrative departments of the county and district people's governments, and the qualifications of teachers in senior middle schools and secondary vocational schools are determined by the education administrative departments of the municipal people's governments with districts.

Applicants can apply for the qualification of teachers in primary and secondary schools and kindergartens at their domicile, residence (local residence permit is required and within the validity period) and place of study (fresh graduates and graduate students only, including students from Hong Kong, Macao and Taiwan); Hong Kong, Macao and Taiwan residents who hold residence permits for Hong Kong, Macao and Taiwan residents can apply for the qualification of teachers in primary and secondary schools and kindergartens in Shanxi Province; Hong Kong and Macao residents hold Hong Kong and Macao residents' passes to and from the mainland, and Taiwan Province residents hold mainland travel permit for taiwan residents valid for five years to apply for the qualification of primary and secondary schools and kindergartens where the teacher qualification examination is held.

Applicants who cannot be verified must provide the original "China Higher Education Certification Report" issued by the National College Students Information Consultation and Employment Guidance Center of the Ministry of Education when confirming on site; Holders of Hong Kong, Macao and Taiwan academic qualifications should provide the original certificate of Hong Kong, Macao and Taiwan academic qualifications issued by the Study Abroad Service Center of the Ministry of Education. Holders of foreign academic qualifications are required to provide the original certificate of foreign academic qualifications issued by the study abroad service center of the Ministry of Education.

(4) Putonghua Grade Certificate

If the applicant's Putonghua proficiency test information has passed the "Teacher Qualification Management Information System", it is not necessary to submit the original Putonghua certificate. If it fails to pass the verification, you need to provide the original Putonghua certificate.

(5) Physical examination certificate

To apply for the qualification of kindergarten teachers, the original of the Physical Examination Form for Kindergarten Teachers in Shanxi Province shall be submitted, and other applicants shall submit the original of the Physical Examination Form for Kindergarten Teachers in Shanxi Province, and shall have a physical examination in the hospital designated by the accreditation body, which will be valid at that time.

(6) No proof of criminal record

Mainland applicants do not need to submit personal proof of no criminal record. The applicant's proof of no criminal record shall be verified by the teacher qualification accreditation institution to the public security organ.

(2) Hong Kong, Macao and Taiwan residents applying for the qualification of primary and secondary school teachers need to provide proof of no criminal record. The certificate of no criminal record shall be issued by the applicant to the relevant departments of Hong Kong Special Administrative Region, Macao Special Administrative Region and Taiwan Province Province. If the applicant needs the assistance of the education administrative department to provide a letter, it shall contact and issue it with the guidance center for teacher qualification identification of Shanxi Provincial Department of Education.

(7) Recent 1 inch bareheaded half-length front color photo 1 piece (official certificate photo, used to apply for teacher qualification certificate, should be consistent with the photo uploaded when registering online, and the back of the photo should indicate the name and the last 4 digits of the ID number).

Verb (abbreviation of verb) matters needing attention

(a) the county education bureau should be identified in strict accordance with the conditions, procedures and authority stipulated by laws and regulations, and it is strictly forbidden to identify the qualifications of teachers for those who do not meet the conditions. If there is any violation, once verified, the parties concerned will be dealt with seriously. All issues involving laws, regulations and policies should be reported in a timely manner and dealt with seriously, and policies should not be broken without authorization.

The relevant materials (original) of the teacher qualification applicant shall be preliminarily examined, and the on-site confirmer shall carefully check the applicant's information, and the various forms downloaded by the applicant must be consistent with the online information. The acceptance form can only take effect after it is signed by the on-site confirmer and stamped with the official seal of the confirmation point.

(2) The deadline for the qualification of teachers in the whole city is June 10, and the county and district education bureaus must complete the qualification of teachers in junior high schools and below before June 10. Before June 13, the county and district education bureaus will submit the roster (electronic version and paper version) for the qualification of teachers below junior high school to the Personnel Department of the Municipal Education Bureau, and at the same time, submit a copy of the Application Form for the Replacement of Teacher Qualification Certificate (with the official seal of the accreditation body) for those who need to reissue the teacher qualification certificate.
