Cancer patients, especially those with advanced cancer, can still talk about rehabilitation? Is it true?/You don't say. /You don't say.

This sentence reminds me of some of my recent patients.

One is a 70-year-old patient with lung squamous cell carcinoma. When he came to our department on 20 17 because of severe pain, the tumor had metastasized to pleura and ribs, which progressed after targeted therapy, and the effect was not good after particle implantation. Taking 160mg morphine every day can't control the pain, because lung cancer is complicated with infection, malnutrition and severe cough, and subsequent treatment can't be carried out because of poor health. After coming to our department, patient-controlled analgesia was used to control the pain first, and then anti-infection and supportive treatment were used. After the pain control, infection control and physical condition are basically restored, contact the internal medicine department, immunotherapy department and radiotherapy department for consultation, take a biopsy again, and make a new tumor treatment plan according to the results of the second biopsy. The treatment effect is good, and the tumor shrinks quickly. However, during the treatment, pain aggravation and interstitial pneumonia appeared intermittently. After adjusting drugs, nerve blocking and symptomatic support, we all recovered very satisfactorily. By the time the tumor was reexamined on 20 19, all aspects of the body reached normal standards and the tumor was basically inactive. The pain is gradually relieved, and the analgesic drugs are gradually reduced. However, due to the long-term use of large doses of opioids, the elderly have a withdrawal reaction during the reduction process. Through drug adjustment and guidance, and the efforts of the elderly themselves, we completely stopped using all analgesic drugs. Recently, his tumor activity has completely disappeared, and he lives like a normal person without any painkillers. During this time, he was busy building himself a villa in his hometown. This is tumor recovery.

The other is an 89-year-old painter with bone metastasis of prostate cancer. Last July, he was hospitalized because of lung infection, pain and confusion. It can be said that he was dying when he came: in addition to tumor progress, infection and pain, the patient also had serious electrolyte disorder because he didn't eat for several days: blood potassium was 2.8, and blood sodium 104 (this is the first time I have seen such an abnormal value). But after our careful treatment, the patient's general condition gradually recovered. It took more than ten days to reach the emission standard. Later, we adjusted the tumor treatment plan through relevant experts and gave follow-up treatment suggestions. More than a year has passed. What's his current situation? The tumor control of the elderly is stable, and they have completely stopped taking painkillers and can take care of themselves. He is happily celebrating his 90th birthday, and his quality of life is much better than that of his wife (who is a sequela of cerebral infarction and can't take care of herself at all). This is also tumor rehabilitation.

Another patient, who was in his fifties more than two months ago, came to the hospital because of lung cancer. He was pulled by an ambulance when he came. On the whole, his condition is extremely poor. Lung cancer spread widely, his lungs were infected and he had difficulty breathing. According to his illness, if he is not treated, it is estimated that his life expectancy will not exceed one month. But his condition is very troublesome: the tumor is widely metastasized, his physical condition is too poor, he has no chance of surgery, and he can't stand chemotherapy and radiotherapy at all. After discussion, our department finally formulated an accurate treatment plan: First, strengthen anti-infection treatment and increase nutrition, so as to improve the patient's condition as soon as possible and enable him to tolerate subsequent treatment; Then, carefully apply the advanced treatment scheme that patients are most likely to tolerate and most likely to be effective at present: immunotherapy combined with targeted therapy. After our combined treatment, the patient has undergone miraculous changes: he used to lie in bed all day taking oxygen, the infection was controlled and his physical condition gradually improved. When he came home, he was completely deprived of oxygen and got up normally. For the second time, the patient came to the hospital on foot; For the third time this month, the patient drives his own car! Review the tumor: it has shrunk by more than half. According to the current treatment judgment, the patient has the possibility of recovery! This is still a tumor recovery.

There are many such examples in our department.

In fact, tumor treatment is like a large-scale battle, which requires all-round efforts to achieve the final victory. To win the war, it is very important for generals to command properly, soldiers to be excellent, and weapons to be excellent, but building bridges, supplying grain and grass, and field hospitals are absolutely indispensable. In the treatment of tumor system, the treatment decisions of patients, family members and doctors are equivalent to those of generals in war; Good therapeutic drugs and technical means are equivalent to excellent soldiers and excellent weapons; Tumor rehabilitation treatment is equivalent to paving roads and bridges, food and grass supply, treatment of the wounded and other logistical support. Tumor rehabilitation treatment refers to the treatment of tumor-related diseases (such as pain and malnutrition) and the control of related symptoms (such as infection, fatigue, edema, hydrothorax and ascites, gastrointestinal dysfunction, etc.). In the process of tumor treatment, various technical means are used to restore the physical condition of tumor patients and improve their quality of life, so as to better carry out subsequent tumor treatment. This is the significance of tumor rehabilitation treatment.
