(2) the original and photocopy of the graduation certificate. (3) a certificate issued by the probation institution that the probation period has expired for one year and passed the examination. If the probation institution is a medical institution, a copy of the probation institution's practice license shall be submitted. (4) If an assistant practicing physician applies for the examination of practicing physician, it shall also submit the original and photocopy of the qualification certificate and practice certificate of the assistant practicing physician, the practice time and the certificate of passing the examination. (5) Traditional medicine teachers or those with special skills who are currently applying for the doctor qualification examination in 2065438+2009 need to submit the following materials: traditional medicine teacher training certificate or traditional medicine skill certificate, traditional medicine teacher and those with special skills qualification examination certificate.
