Wuxi Disability Certificate Application Process

1, application

In accordance with the principle of voluntary licensing, I or my guardian shall apply to the Disabled Persons' Federation in the community (village) or street (township) where the household registration is located with the household registration book, ID card, two two-inch color bareheaded recent photos and relevant medical records, and fill in the Information Collection Form for Disabled Persons as required, and the community (village) and neighborhood committee will conduct a preliminary examination of the applicant.

Step 2 identify

The applicant shall go to the designated hospital for disability assessment and testing with the disability assessment form returned by the municipal (county) and district disabled persons' Federation, the letter of introduction of Wuxi disability testing and relevant medical records.

Step 3 evaluate

The designated institution shall assess the disability of the applicant who applies for the Disabled Persons' Card, make a clear conclusion on the category and level of disability according to the disability standard, fill in the assessment form and affix the official seal.

4. Joint review

The office of the Municipal Committee for the Assessment of Disability organizes experts to conduct joint examination on the assessment and test results of disability, and the office for the assessment of disability will input the joint examination results into the database, and automatically generate file numbers for applicants who meet the identification conditions, and affix official seals to issue disability certificates to the District Disabled Persons' Federation.

Step 5 distribute

District Disabled Persons' Federation issues disability certificates to street (township) disabled persons' federations, and street (township) disabled persons' federations issue disability certificates to community (village) neighborhood committees, and disabled persons receive disability certificates from community (village) neighborhood committees.
