There are many reasons for the accident. 1) Equipment failure is the direct cause; 2) There has been no overhaul in recent four years, and hidden dangers already exist; 3) design problems, which may cause the workpiece to get stuck; 4) The operator illegally holds the card.

Cause of accident: 1) freshmen should not operate alone; 2) No safety protection device is used.
