Recently, some residents in our district have changed the code of Luoyang electronic health code. In order to do a good job in the people's health management services in the region in an orderly manner, according to the requirements of the Notice of Luoyang COVID-19 Epidemic Prevention and Control Headquarters Office on Changing the Rules of Health Codes, we hereby notify the residents of our region about the transcoding of Luoyang electronic health codes:

Persons whose health code is yellow may apply for change according to the following provisions if they have any objection to the code allocation.

I. Proof materials

1. Apply for changing Luo's health code.

2. Extract the trip code from the mobile phone and print it out.

3. Negative nucleic acid certificate issued by medical and health institutions within 48 hours.

Two, change the code of personnel nucleic acid sampling point

In order to facilitate code changers to carry out nucleic acid testing at various sampling points and reduce the risk of epidemic spread, the First Affiliated Hospital of Heke University of Science and Technology set up a new sampling point for nucleic acid testing in the bicycle hall of Luoyang Sports Center on the 7th, mainly providing nucleic acid testing and sampling services for "yellow code" personnel who need to eliminate the risk of nucleic acid testing. The newly set nucleic acid detection point is located at the intersection of Taikang Road and Xinyue Road in Luolong District. People who need to be tested can queue up at the west gate of the bicycle hall for nucleic acid testing. Nucleic acid detection and sampling service is carried out at the inspection site from 9:00- 17:00 every day, with no rest on weekends.

The "yellow code" personnel who go to this point for nucleic acid detection need to provide their ID card, health code and "proof" stamped with the official seal of the community where they live to sample at the sampling point. The "certificate" is handled by family members or friends. Those who meet the transcoding conditions will be distributed by the community according to the actual situation of the personnel.

"Yellow code" personnel are not allowed to take public transport when they go to the sampling point for nucleic acid detection. They must wear masks, keep a "one-meter line" and take personal protective measures, while paying attention to cold and warmth.

Third, change the procedure.

1. Personnel in Xigong area are entrusted by family or friends. Please submit the above proof materials to the leading organization of epidemic prevention and control in the street where you are located, and the information officer in charge of nailing system at the same level will change the health code.
