Fire protection product supply certificate

Apply to the fire station for the supply certificate of fire-fighting products, and provide information: 1, information form of fire-fighting products business unit (signed by the legal person and stamped with the official seal of the unit);

2, the manufacturer (and agent or branch) business license (copy with official seal);

3. Official sales authorization document signed and sealed by the legal representative of the manufacturer;

4. A power of attorney signed and sealed by the legal representative of the enterprise and a copy of the licensee's ID card;

5. Copies of production qualification certificates such as certification or type approval certificate and test report shall be stamped with the official seal of the manufacturer.

Note: The above copies need to be checked with the original; The handling place is the office of the product inspection department.

According to the Reply of the Provincial Price Bureau to the Provincial Information Industry Department on the charging items and standards of electronic authentication services (Guangdong Price Letter [2008] No.649), each enterprise is required to charge an annual digital certificate service fee of 400 yuan, one-time electronic key fee 150 yuan.
