(two) written materials of labor arbitration

1. Apply for arbitration

The contents include the basic information of the parties (name, sex, date of birth, ID number, address, mailing address and telephone number of the workers; Name, domicile, mailing address, telephone number and name and position of the legal representative or principal responsible person of the employing unit), specific arbitration request and amount, facts and reasons on which it is based, evidence (photocopy) and sources of evidence, and name and domicile of witnesses.

In addition to the original application and evidence, copies should be submitted according to the number of applicants. The application for arbitration shall be signed by the applicant himself (if there is a ghostwriting, it shall be marked and sealed by him for confirmation).

Article 9 of the Organizational Rules for Arbitration of Labor and Personnel Disputes stipulates that the Arbitration Commission shall set up a practical office to undertake daily work such as dispute mediation and arbitration. The office is called Labor and Personnel Dispute Arbitration Institute, which is located in the administrative department of human resources and social security. The arbitration tribunal shall be responsible to the arbitration commission and report its work.
