First, the scope of appraisal

(2) Persons registered in other provinces and cities who hold residence permits in Potou District and are within the validity period;

The above-mentioned personnel who meet the admission requirements before May 3, 2065438 (the admission time is subject to the admission time on the diploma) and are full-time normal students in preschool normal schools, full-time normal students in colleges and universities and full-time masters (doctors) in education can be directly recognized as teachers corresponding to their majors and study periods. Other personnel applying for the qualification of primary and secondary school teachers need to hold the certificate of qualification examination for primary and secondary school teachers issued by the examination center of the Ministry of Education within the validity period, and the "teaching subjects" applied for should be consistent with the interview subjects.

Second, the identification conditions

Those who apply for the accreditation of teachers' qualifications shall meet the accreditation conditions stipulated in the Regulations on Teachers' Qualifications and have not reached the national statutory retirement age.

(A) ideological and moral status

(2) Education

1. To apply for kindergarten teacher qualification, you must have graduated from kindergarten normal school or above.

2. To apply for the qualification of a primary school teacher, you must have graduated from a secondary normal school or above.

3. To apply for junior high school teacher qualification, you must have a higher normal college or other college degree or above.

4. To apply for the qualification of a senior middle school teacher, you must have a bachelor's degree or above from a normal university or other university.

5. To apply for the qualifications of teachers in secondary vocational schools (including the qualifications of teachers in secondary vocational schools, technical schools, vocational high schools for cultural courses and professional courses), you must have a bachelor's degree or above in normal universities or other universities.

6. To apply for the qualification of internship instructor in secondary vocational schools (including the qualification of internship instructor in secondary vocational schools, technical schools and vocational high schools), you must have a secondary vocational school graduation or above, and at the same time, you should have a junior and above professional technical position or a technical grade certificate for intermediate and above workers.

(3) Physical condition

Have good physical and psychological qualities, no infectious diseases, no history of mental illness, and can meet the needs of education and teaching. (2) "Teacher Qualification Examination Information". Applicants who have participated in the national primary and secondary school teacher qualification examination and passed the examination can view their examination certificate information in this column.

(3) "Putonghua certificate information". Applicants can add and modify personal Putonghua information in this column.

(4) "Education and student status information". Applicants can add and modify personal academic information under this column. ③ If the certificate information cannot be verified through the above steps, please select the type of "unable to verify academic qualifications", fill in the relevant information and upload the corresponding electronic certificate (the picture size is less than 200KB and the format is JPG) for background manual verification.

④ For secondary school education such as normal school and kindergarten teacher, please select the type of "education that cannot be verified", fill in the relevant information and upload the corresponding electronic certificate (the picture size is less than 200KB and the format is JPG) for manual verification in the background.

⑤ If your education is from Hong Kong, Macao and Taiwan or studying abroad, and you can't verify your education, please select the verification type as Hong Kong, Macao and Taiwan education or studying abroad, and upload the certificate of Hong Kong, Macao and Taiwan education or foreign education according to step 3.

Applicants can download personal books in the "Information Download" column or "Remarks" page on the homepage of "China Teacher Qualification Network" before the registration is confirmed, and use them when registering.

Note: The downloaded personal books are printed on A4 white paper. If you use this book in the application form of teacher qualification certificate, please write it in block letters at the "person" and sign my name. After filling in the signing time at the "year, month and day", take a photo vertically and clearly on the front of the paper and upload it. (2) Registration

After the online registration of primary and secondary school teachers' qualifications in Guangdong Province began, the applicant registered on the "China Teacher Qualification Network" with his account number within the specified online registration time.

1. Select a certification authority

The accreditation body and on-site confirmation point for applying for online registration of kindergarten, primary school and junior high school teachers' qualifications is the Potou District Education Bureau of Zhanjiang City.

Those who apply for the qualifications of senior middle school teachers, secondary vocational school teachers and practice instructors must choose Zhanjiang Education Bureau, which will be named Potou District Education Bureau after on-site confirmation.

2. Online registration (20201October 9 10 to June 6 10)

(3) On-site audit

1. Identification

Applicants who hold academic degree certificates of institutions of higher learning in Hong Kong, Macao and Taiwan must provide the original academic degree certification certificate issued by the Study Abroad Service Center of the Ministry of Education, and the academic degree certification certificate shall be regarded as valid; Applicants who hold academic degree certificates of foreign institutions of higher learning are required to provide the original foreign academic degree certificates issued by the study abroad service center of the Ministry of Education, and the academic degree certification certificate is deemed to be valid.

3. Putonghua Grade Certificate

If the applicant's Putonghua proficiency test information has been verified at the time of registration in "China Teacher Qualification Network", it is unnecessary to provide the original Putonghua certificate. If it fails to pass the verification, you need to provide the original Putonghua certificate.

Special note: the applicant can't find the results, certificate collection, certificate replacement and other issues on the national Putonghua training and testing information resource network (website: http://www.cltt.org/studentscore). 4. Physical examination certificate

Applicants should go to Zhanjiang Central People's Hospital, the Affiliated Hospital of Guangdong Medical University, the Second Affiliated Hospital of Guangdong Medical University (Zhanjiang Second People's Hospital) or Aizhou Hospital in Potou District for physical examination before on-site confirmation, and obtain the physical examination certificate issued by the hospital.

5. Proof of no criminal record

(1) Mainland applicants do not need to submit personal proof of no criminal record. The applicant's proof of no criminal record shall be verified by the teacher qualification accreditation institution to the relevant departments.

(2) Hong Kong, Macao and Taiwan residents applying for the qualification of primary and secondary school teachers need to provide proof of no criminal record. The certificate of no criminal record shall be issued by the applicant to the relevant departments of Hong Kong Special Administrative Region, Macao Special Administrative Region and Taiwan Province Province. The Letter of Proof of No Criminal Record (Annexes 2 and 3) required by the applicants of Hong Kong Special Administrative Region and Macao Special Administrative Region shall be filled in by the applicants and submitted to the on-site confirmation point. After the provincial organization fills in the seal, it informs the applicant to collect it from the confirmation organization, and then the applicant applies for a certificate of no criminal record with the letter to the police department in Hong Kong or Macao. After verification, the Hong Kong and Macao Police Department will feedback the verification results to the provincial organization that issued the letter, and the provincial organization will notify the relevant education bureau as the confirmation basis. The applicant's certificate of no crime in Taiwan Province Province shall be issued by the applicant to the relevant departments of Taiwan Province Province, and the certificate of no crime issued shall be directly handed over to the accreditation institution applying for accreditation.

6. A recent 1 inch bareheaded photo with a color white background.

Special note: recent bareheaded 1 inch color photos with a white background (upload format is JPG/JPEG, not exceeding 200K) are uniformly used for photos uploaded by the information system and photos pasted by teachers' qualification certificate holders.

7. To apply for the qualification of internship instructor in secondary vocational schools, in addition to providing the above information, the original professional and technical post certificate or worker's technical grade certificate shall also be provided.

8.2065438+Full-time normal education graduates who entered the school before May 3, 20061apply as normal students to be directly recognized as teachers corresponding to their majors and periods of study, and must provide academic transcripts (with achievements in pedagogy, psychology and internship) stamped by the school and a copy of the internship appraisal form.

(four) to obtain a teacher qualification certificate.

After the accreditation body completes the on-site audit, it will make a review conclusion according to the audit situation, and make a teacher qualification certificate for the applicants who meet the review conditions. After receiving the notice, the applicant will receive the teacher qualification certificate and the teacher qualification application form at the acceptance point with his ID card (the teacher qualification application form must be submitted by the applicant to the management department where my personnel file is located, and it will be included in my personnel file at his own risk).

After graduation, fresh graduates need to hold their ID cards and academic certificates to receive teacher qualification certificates and teacher qualification application forms at the acceptance point.

The time and place to receive the teacher qualification certificate and the teacher qualification application form shall be subject to the notice of the accreditation body. Please keep the communication normal and pay attention to the notice issued by the certification body in time.

Fourth, other matters needing attention

(a) the applicant shall personally make online registration and on-site audit confirmation, and be responsible for the accuracy and authenticity of the personal information and on-site audit materials provided. No school or any institution is allowed to register for the exam. If my application for teacher qualification is affected by others' substitution for registration, the responsibility shall be borne by the applicant himself.

(2) The "Teacher Qualification Management Information System" automatically verifies the applicant's identity, education background, Putonghua, primary and secondary school teacher qualification examination results and other information, and the applicant can only pass the verification of the above information by filling in real personal information.

(3) The photos uploaded by applicants during online registration should be consistent with those submitted during physical examination and on-site audit, and meet the requirements. If the unqualified photos affect my application for teacher qualification certificate, the responsibility shall be borne by the applicant himself.
