4. Provide the original and photocopy of my ID card (2 copies), Shenzhen labor security card or Shenzhen employee social insurance card.

3. If there is no social insurance certificate, a recent one-inch color photo shall be affixed to the application form, and the official seal of the unit (riding seal) shall be affixed.
