Where can I report the loss of Zibo medical insurance card? How to reissue it

At present, the function of medical insurance card has been replaced by social security card.

After the cardholder's social security card is lost, it should be reported in advance to prevent the loss of funds in the medical insurance account in the social security card. Cardholders can call the social security card service hotline "12333" (24-hour service) in advance to report the loss. You can also go to the social security card service outlets with your resident ID card or residence booklet to handle the written pre-loss reporting procedures, or directly report the loss at the social security card self-service terminals set up by the social security card service outlets and designated medical institutions at or above the second level. The effective time of pre-loss reporting is 5- 10 days, and it will be automatically released after the effective time limit.

After pre-reporting the loss, if the loss-reporting person determines that the social security card is lost and can't be retrieved, he/she needs to present his/her resident ID card to the social security card service outlets to formally report the loss, and at the same time, he/she should go through the formalities of replacing the card and pay the card-making fee. Generally, within 30 working days, the applicant will go to the social security card service outlets to receive a reissued social security card with his/her resident ID card or residence booklet and the Social Security Card Renewal Certificate (hereinafter referred to as the "Card Certificate").

After the formal loss reporting, the loss reporting cannot be cancelled.
