1. How to apply for a social security card for the first time?

(1) Those who have reached the age of 16 shall apply at any social security card center or service outlets in this city with valid certificates. Need photos.

(2) Minors under the age of 16 shall be handled by guardians with their valid certificates and guardian ID cards. Don't need photos.

Valid documents include: the second-generation identity card of residents, the passport or permanent residence permit of foreigners, and the pass for compatriots from Hong Kong, Macao and Taiwan to and from the mainland.

2. What should I do if I lose my social security card?

If the social security card is lost, it should be reported as soon as possible through one of the following ways:

(1) Call 12333. If 12333 answers the service manually, it will also report the loss of the bank card orally; 12333 self-service voice service can only report the loss of social security cards, and cannot report the loss of bank cards in linkage.

(2) Report the loss of the social security card at the social security card center counter or service outlets. After reporting the loss at the counter, the bank card will be reported orally.

(3) Report the loss in writing at the bank counter where the social security card is opened. After the bank card is reported in writing, the social security card will report the loss in linkage.

3. How to reissue the lost card?

After the social security card is damaged or reported lost, you can apply for a replacement card:

(1) I reported the loss of my social security card in writing at the corresponding bank outlets with my ID card and obtained a blank card;

(2) Change the card at the card center with a blank card.
