Instructions for labor ability appraisal

Please provide the following materials:

1. Fill in the application form for labor ability appraisal;

2. A copy of the appraiser's ID card;

3. Original medical records of the appraiser (psychiatric patients need more than two years' medical records), medical information and recent diagnosis certificate (medical appraisal certificate issued by qualified hospital experts);

4. Other information that can objectively reflect the injury (illness) situation of the assessed person (such as shooting of the injured part and inspection data, etc.);

5, because of work-related injuries and occupational diseases to apply for labor ability appraisal, provide the administrative department of labor and social security to make the "work-related injury identification decision".

6. Take a recent photo of one inch.

Units or individuals applying for appraisal are not satisfied with the conclusions made by the Municipal Labor Ability Appraisal Committee, and may apply for re-appraisal within 0/5 days from the date of receiving the appraisal conclusion.

After the conclusion of labor ability appraisal is made 1 year, if the appraised employees or their immediate family members, their units or agencies think that the disability situation has changed, they may apply for re-appraisal of labor ability.
