The annual social insurance premium settlement declaration form is as follows:

City and district payment enterprises:

This declaration form is used for the annual settlement of enterprise social insurance premiums of the paying enterprises, and only for the payment (on-the-job) part of the social insurance premiums paid by the paying enterprises. There is a main table and two schedules in the declaration form. Please read the instructions carefully before filling out the form.

1, according to the "total wages of all employees", the order of reporting by enterprises is: Schedule 1→ Main Table;

2. The filling order of the labor service enterprises of ship outsourcing engineering and some construction enterprises that choose to settle with "operating income" is: Schedule 2→ Schedule 1→ Main Table.

First, the main table

1. When applying online, the main table is automatically extracted and calculated by the computer except for supplementary data. Supplementary information shall be truthfully filled in by the payment enterprise. If there are any unfilled items, they cannot be declared successfully, and return to the operation according to the prompt.

2. The data of the total wages in column 4, the amount of items deducted in column 5, the payment base in column 6 and the amount payable in column 7 in the table are respectively extracted from the corresponding expenses in columns 1 4, 15 and 17 of the attached table, and calculated by expense division.

3. The amount declared in the eighth column of settlement period is generated by extracting database information from the computer according to the charging item, and the access caliber is the tax payable in this settlement period that the charging item has been successfully declared to the local tax department; If there is a refund amount in this settlement period before the annual settlement declaration, it will be deducted.

4. Column 9 in the table, in which: the reduced or paid-in amount is generated by extracting the database information from the computer according to the charging items, and the retrieval caliber is the total reduced or paid-in amount during the charging item settlement period (including the start and end dates) (centralized reduced amount in March 2009).

5. 10 column of the amount approved during the settlement period but not reduced or offset is generated by extracting database information from the computer according to the expense items, and the value range is the amount that has not been reduced or offset during the expense settlement period (including the start and end dates) that has been approved according to the reduction or offset process during the settlement period.

6. After the enterprise successfully declares this form, the number of applicants will be generated according to the column of "Amount of Fees to be Paid (Refund)"1,and then it will enter the collection process.

Two. Schedule 1

1 and schedule 1 are the schedules of the main table. When an enterprise makes a settlement according to the "total wages of all employees", it must first fill in the attached table 1.

2. Shipping outsourcing enterprises and some construction enterprises that choose to settle accounts according to "operating income" must fill in Schedule 2 first, leaving column 4- 14 blank. The payment base in column 15 is taken from column 8 of Schedule 2 according to the cost.

3, the fourth column "the total wages of all employees in this period" fill in the total amount of labor remuneration paid directly by the enterprise to all employees of this unit during the settlement fee period, and the total wages of all types of insurance should be consistent.

4. The seventh column "individual contribution base of the insured inside and outside the city" refers to the individual contribution base of the insured outside the city and the insured of other units in the city, except for work-related injury insurance, which is issued by the social security agency outside the city or inside the city with relevant valid certificates and audited by the competent local tax authorities.

5. The eighth column "individual contribution base of employees participating in government agencies and institutions" refers to the individual contribution base of employees participating in government agencies and institutions in this city, which is allowed to be deducted from the total wages of all employees after relevant valid certificates are issued by local social security institutions and audited by the competent local tax authorities.

6. Column 9 "wages of foreigners" refers to the total wages paid by enterprises to foreigners, Hong Kong, Macao and Taiwan personnel.

7. 10 column "Wages of personnel participating in provincial overall planning" refers to the total wages of employees participating in provincial overall planning in the industry, which can be deducted from the total wages of all employees after the relevant valid certificates are issued by the provincial social security management center and audited by the competent local tax authorities (uninsured insurance cannot be deducted).

8. 1 1 column "Deductible wages of migrant workers" is only filled in by the construction enterprises that settle accounts according to "total wages of all employees", and can only be deducted from the total wages of all employees when calculating work-related injury insurance premiums. Deductible wages of migrant workers = total project cost × 15%× (total days of project in settlement year/total days of contract period). Other types of insurance cannot be deducted.

9. 12 column "Adjustment of sum difference of annual cumulative individual contribution base" is used to adjust the difference between the minimum declaration base calculated according to the minimum declaration ratio and the sum of annual cumulative individual contribution base:
