Chengdu's separate bid is divided into two situations according to the different applicable groups, mainly in the lots.

2. Insured persons participating in the endowment insurance for urban and rural residents and adult residents and scattered children participating in the basic medical insurance for urban and rural residents. Its handling place is in the street (township), community (village) labor and employment social security service center (station) or other designated social security card service outlets where the household registration has been announced.

Applicants need to bring their original valid identity documents to the processing place to receive the Chengdu Social Security Card Application Registration Form, and check and fill it out. At the same time, the registration form must be affixed with a copy of the applicant's valid identity certificate;

If the photo is not printed on the application form or needs to be replaced, the applicant must paste a two-inch recent bareheaded front photo, a dark coat and a color certificate on a white background.
