Each local regulation is different, it is recommended to consult the local social security center 12333. In Shanghai, for example, according to the "Shanghai Social Security Card Management Measures" regulations:

Article 11 (Application)

People who have a permanent household registration in the city and who have reached 16 years of age can apply for a social security card.

Those who apply for a social security card shall go to the social security card application point in the street (township) where their hukou is located to go through the following procedures:

(1) filling out the registration form for application for a social security card in Shanghai;

(2) handing over and verifying the resident's identity card and resident's hukou book or proof of hukou registration;

(3) going to the designated place to provide the pictorial information.

Article 12 (Production and Issuance of Cards)

Social security cards are uniformly produced by the Municipal Information Service Center in accordance with Shanghai's local standard DB31/T256-2000 "Social Security Cards", and personal information is recorded.

Social security card application points in each street (township) are entrusted by the Municipal Information Service Center to specifically handle the issuance of social security cards.
