(3) Materials required for the change of unit information:
1. If the name and nature of the unit change, the original and photocopy of the new certificate (copy of legal person certificate or business license or copy of organization code certificate or registration certificate), the Registration Form for Information Change of Housing Provident Fund Units in duplicate and the certificate of change issued by the industrial and commercial department are required.
2. If the legal representative or person in charge changes, provide the original and photocopy of the new certificate (copy of legal person certificate or business license or copy of organization code certificate or registration certificate), copy of the registration form for information change of housing provident fund unit, and copy of the new legal person ID card (stamped with the official seal or filing seal of the unit).
3. If the unit manager is changed, provide a copy of the Registration Form for Information Change of Housing Provident Fund Units, a copy of the new power of attorney and a copy of the ID card of the new manager (and provide the original for checking). If the original manager is also the digital certificate authorizer, the digital certificate key must be provided.
5. When the source of funds changes, the unit shall provide a copy of the certificate of the department that approved the change of the source of funds of the unit (and provide the original for verification) and an Application Form for the Change of Source of Funds of the Unit in duplicate.
Legal basis: Article 57 of the Social Insurance Law of People's Republic of China (PRC). If the social insurance registration items of the employer are changed or the employer terminates according to law, it shall go to the social insurance agency to change or cancel the social insurance registration within 30 days from the date of change or termination.
