(1) Supplementary certificate: If the outpatient medical certificate for special diseases is lost or damaged and cannot be used, you can apply for a replacement, and you need to provide: the social security card (medical insurance card) of the insured patient; ID card and photocopy of the patient or handler; Apply for a replacement certificate; Recent 1 1 inch photos of patients.

(2) Increase of diseases: If the insured who has obtained the qualification of serious diseases in outpatient department has serious complications, or the newly added diseases meet the prescribed standards, it is necessary to apply for increasing diseases and re-approve the quota standard. For all kinds of complications of hypertension and diabetes, as long as the patient's condition reaches the prescribed standard, the disease type is approved as "hypertension complicated with heart, brain, kidney and vascular complications" or "diabetes complicated with heart, brain, kidney, eye and neuropathy", and the corresponding limit is approved at one time without increasing the disease type; Malignant tumor metastasis, metastasis treatment does not need to increase the number of diseases; If uremic dialysis patients have uremic complications such as hypertension and myocardial ischemia, there is no need to increase the disease. The application materials and approval procedures for adding diseases are the same as the first time.

(3) Re-apply for a certificate: For the insured who has been disqualified from outpatient serious diseases, it is really necessary to re-apply for the Special Disease Outpatient Medical Certificate. The application materials and approval procedures are the same as the first time.
