1, application conditions

Persons participating in medical insurance for urban and rural residents who suffer from prescribed diseases may apply for special disease treatment.

2. Disease scope: malignant tumor, anti-rejection therapy after organ transplantation, organ failure, cerebrovascular accident, hypertension, diabetes, aplastic anemia, hemophilia, severe mental disorder, systemic lupus erythematosus, chronic viral hepatitis, Parkinson's disease, tuberculosis, childhood autism, and complete growth hormone deficiency.

Step 3 provide materials

1) Medical insurance card and ID card for urban and rural residents (copy)

2) Approval form for special diseases stamped by the medical insurance department of designated medical institutions above the second level.

3) discharge summary or outpatient medical records

4) Examination, inspection, image report and other materials related to the diagnosis of diseases.

4. Application process

1) The insured person shall go to the social security agency, the designated medical institution or the township (street) agency to receive the Application Form for Special Disease Outpatient of Medical Insurance for Urban and Rural Residents.

After the chief physician of a medical institution above the second level fills in the "main medical history", "course of treatment" and "providing evidence", the director of the department signs the "diagnosis opinion", and the medical insurance department of the designated medical institution agrees to seal it, the insured person will send the special disease outpatient examination and approval form and the relevant materials for applying for special diseases to the township (street) agency, and the township (street) agency personnel will register and summarize the materials and submit them to the urban medical insurance department of the county social security bureau.

2) The county social security bureau shall set up a special disease identification team to review the application of the insured person on 25th of each month, give approval to the insured person who meets the conditions of special disease identification, issue a special medical record for special disease outpatient service, and return it to the declaration point of the insured place in a unified way, and the applicant will collect it at the insured place.

3) When examining and approving medical treatment, patients with special diseases must hold special medical records and medical insurance cards for medical treatment according to regulations. They can report directly by swiping their cards when they are in hospital. Medical expenses unrelated to the designated special diseases shall be reimbursed in accordance with the relevant provisions of the general outpatient department. Outpatient expenses for special diseases generated by designated medical institutions at or above the second level outside the county shall be accepted by the township (street) agency by the outpatient invoice (original), medical insurance card, special medical records for special diseases, credit cooperative card or passbook (copy).

5. Processing time limit

The conditions are met and the materials are complete, which is generally completed within 30 days.
