Legal basis: Article 7 of the Notice of Ministry of Human Resources and Social Security on Printing and Distributing the Measures for the Administration of Social Security Cards of the People's Republic of China shall submit the following materials when applying for the issuance of social security cards: (1) Fill in a complete application form for the issuance and registration of social security cards; (2) An explanation of the preparation for the issuance of social security cards; (3) The application fields and application file structures in the card that meet the requirements of the national unified specifications (including the application fields and indicators of local expansion); (four) the card face style designed according to the unified requirements; (five) the measures for the administration of social security cards in this area and the detailed rules for their implementation; (six) other materials required by Ministry of Human Resources and Social Security.
