③Proof of the time of abortion or induced miscarriage issued by the medical institution.

(3) Miscarriage or induced abortion after unplanned pregnancy and having family planning surgery or revision surgery. Please bring the original of the following information:

①Marriage certificate;

②Medical records, discharge summary and hospitalization expenses detailed summary list;

③Medical fee invoices.

(4) Unemployed spouses of male workers are entitled to fixed medical compensation. Please bring the original of the following information:
