Legal basis: Article 7 of the Administrative Measures of the People's Republic of China on Social Security Cards. To apply for issuing social security cards, the following materials shall be submitted:

(1) Fill in a complete social security card issuance registration application form;

(2) An explanation of the preparation for the issuance of social security cards;

(3) The application fields and application file structures in the card that meet the requirements of the national unified specifications (including the application fields and indicators of local expansion);

(four) the card face style designed according to the unified requirements;

(five) the measures for the administration of social security cards in this area and the detailed rules for their implementation;

(six) other materials required by Ministry of Human Resources and Social Security.
