4. Declaration procedures. Eligible employers should apply for social insurance subsidies every May 1 1, and fill in the Application Form for Social Security Subsidies for Persons with Employment Difficulties (Annex 1) and the Roster of Social Security Subsidies for Persons with Employment Difficulties (Annex 2) with the labor contract, employment unemployment registration certificate and other materials.

3. Declaration procedures. Eligible flexible employees shall pay social insurance premiums according to regulations, bring their resident identity cards, household registration books and employment unemployment registration certificates, fill in the Registration Form for Persons with Employment Difficulties in Kunshan City (Annex 3) and the Application Form for Social Insurance Subsidies for Flexible Employees in Kunshan City (Annex 4), apply for employment to the local village committees (neighborhood committees), and go through the application procedures at the municipal employment management department.

4. During the period of enjoying social insurance subsidies, flexible employees will stop enjoying social insurance subsidies under any of the following circumstances.
