The contributing unit shall fill in two copies of the "Tax (Social Security Contributions) Registration Form" and submit the original and one copy of the legal representative's (person in charge's) identity document, the original and one copy of the practice license, and a copy of the tax registration certificate (not required for those who are registered for both tax registration and contributions registration), and then apply to the registration point to apply for registration of social insurance contributions.
