How to fill in the settlement form of enterprise social insurance premium

Fill in the Summary Table of Total Wage Adjustment Items of Social Insurance Premium as follows:

1. This declaration form is used for the annual final settlement of the enterprise social insurance premium of the paying enterprise, and only used for the part paid (on-the-job) by the paying enterprise social insurance premium unit. There is a main table and two schedules in the declaration form. Please read the instructions carefully before filling out the form.

2. The order of reporting by enterprises according to "total wages of all employees": Schedule 1→ Main Table;

3. The filling order of the labor service enterprises of ship outsourcing engineering and some construction enterprises that choose to settle with "operating income" is: Schedule 2→ Schedule 1→ Main Table.

4、? Employees who have worked for one year shall take their average monthly salary in the previous year as their social security payment salary. New employees can use the salary agreed in the labor contract as the payment salary of new employees' social security.
